Data collected
Your survey answers: depending on your path, your company’s practices or your experience of maternity leave, plus your company’s size and industry, your country, any free-text answers and the survey language. Email is optional and only used if you ask for the summary or agree to be contacted. Your name is not asked for. Your answers are confidential but not strictly anonymous: an email identifies you, and a combination of answers can sometimes be enough to recognise someone. Netlify, which receives the form, may record technical data about the submission (date, IP address, browser) [to be confirmed]. Please don’t include colleagues’ names, identifiable medical information or confidential company information in free-text answers.
Purposes
To understand how maternity leave is prepared for, experienced and supported, from both the employee and the employer side, before deciding whether a support project is useful and in what form. Depending on what comes out of the answers, a study may be published and made available to anyone. Results in it are presented in aggregate, with nothing that could identify a person, a company or a small group. Short extracts from free-text answers may be quoted once anything identifying has been removed. No company is named without the explicit consent of the respondent who mentioned it. Your email is only used, depending on your choices, to send you the summary or to contact you for a conversation.
Legal basis
Your consent, given by sending the survey, and your separate choices for the summary and for being contacted. Answers about maternity leave may count as sensitive data under the GDPR [legal basis and possible explicit consent to be confirmed].
Recipients
Claire Connor only. Answers are hosted by Netlify, Inc. (United States), which is certified under the EU-U.S. Data Privacy Framework, its UK Extension and the Swiss-U.S. Data Privacy Framework. No data is sold or shared with third parties. If you book a conversation, booking takes place on Calendly (Calendly LLC, United States) under its own privacy policy.
Retention
[12 months] after the results are published, then emails are deleted; only anonymous statistics are kept.

Your rights
Access, correction, deletion, withdrawal of consent: write to claire.connor.weill@gmail.com. If you left an email, your response can be found and deleted. Without an email, give the approximate date you answered and a few details (path, country, industry): I’ll do my best to find it, but can’t guarantee it. You can also contact your data protection authority (in France, the CNIL, cnil.fr).
